Applicant authorizes the Company to initiate debit and credit entries to the bank account identified on this form for payment of invoices, refunds, and adjustments, and to correct erroneous entries. This authorization remains in effect until Company receives written revocation from Applicant and has had a reasonable opportunity to act on it.

Applicant agrees to pay late fees and finance charges in accordance with the Late Fee / Interest Policy set forth above and to reimburse Company for all reasonable collection costs, including attorneys' fees, incurred in collecting overdue amounts.
