Account Opening and Agreement: The undersigned requests the opening of the account described above and agrees to be bound by the institution's account rules, disclosures and fee schedules as in effect from time to time. Deposits are accepted subject to verification and clearing. The institution may require additional documentation and may refuse or close accounts in its discretion where permitted by law.

Funds Availability: Availability of deposited funds shall be subject to verification and the institution's funds availability policy. The institution may place holds on deposited items in accordance with applicable law.

Overdrafts and Setoff: The account holder authorizes the institution to debit any of the account holder's accounts to cover overdrafts. The institution reserves the right to set off funds in any of the account holder's accounts against liabilities owed to the institution by the account holder to the fullest extent permitted by law.

Amendment and Termination: The institution may amend these terms by notice to the account holder. Either party may close the account with prior written notice as required by applicable law. Termination does not affect liabilities or obligations incurred prior to closing.

Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the state of , without regard to conflicts of law principles.

By signing below, the account holder certifies under penalty of perjury that the information provided in this document is true, accurate and complete, that the account holder is the beneficial owner of the funds or is authorized to act on behalf of the beneficial owner, and that the account will not be used for unlawful purposes. The account holder further certifies compliance with applicable tax reporting and anti-money laundering laws, and agrees to provide documentation reasonably requested to verify identity and beneficial ownership.
