The Account Holder authorizes the financial institution and the payer to initiate electronic credits and debits to the designated account as required for payments, refunds, or transfers. This authorization remains in effect until the financial institution and the payer receive written notice of revocation at least thirty (30) days prior to the requested termination date. The Account Holder represents and warrants that they have full authority to enter into this authorization and to bind the account identified herein. The Account Holder agrees to indemnify and hold harmless the financial institution, payer, and their agents for losses resulting from inaccurate information provided by the Account Holder or delays caused by the Account Holder's failure to provide timely notice of a change.
