By signing this Mandate, the Debtor authorizes the Creditor named above to initiate debits to the specified bank account and instructs the bank to honor such debits. The Debtor certifies that the account details provided are correct and that the Debtor is empowered to operate the account.

The Debtor understands that notification of individual debit amounts and schedule will be provided in accordance with the payment frequency or otherwise in writing at least days prior to the debit, unless otherwise agreed.

The Debtor has the right to request a refund for an unauthorized or erroneous debit in accordance with the terms of the Debtor's agreement with their bank. The Creditor will not be liable for delays or failures caused by the Debtor's bank or by events beyond the Creditor's reasonable control.

Cancellation: The Debtor may revoke this mandate by providing written notice to the Creditor and, where required by the Debtor's bank, by notifying the bank. Revocation will not affect debits already lawfully initiated prior to receipt of revocation by the Creditor or the bank.
