By signing below, Payor authorizes Biller to initiate debit entries, and if necessary, credit entries and adjustments for any debit entries made in error, to the account identified above. This authorization remains in effect until revoked by written notice delivered to Biller at the address provided above, giving Biller at least 10 business days to process the revocation prior to the next scheduled debit.

Payor represents and warrants that Payor is either the named account holder or is expressly authorized by the named account holder to provide this authorization. Payor agrees to notify Biller immediately of any change in account details or termination of authority. Biller is entitled to rely on the account and routing numbers provided; Biller will not be liable for delays or losses caused by inaccurate information supplied by Payor.

In the event a debit is returned or dishonored, Payor agrees to pay any reasonable returned item or processing charges, and accepts that Biller may attempt to re-present the debit. Re-presentment may occur without further notice to Payor where permitted by law. Payor's liability for unauthorized debits is subject to applicable banking rules and consumer protection laws.

Payor acknowledges collection and processing of personal and financial information as necessary to effect transactions under this authorization. Such information will be used in accordance with Biller's internal privacy practices and applicable law for billing, verification and collection purposes.
