Authorizations and Consent

By signing this form, the Client expressly authorizes the Service Provider to assess and collect the fees described above. Client authorizes the Service Provider to recover fees by debiting the specified account or by any contractually permitted method, including set-off against balances, subject to applicable law.

Client authorizes recurring fees where identified as recurring. Client acknowledges that fees may be adjusted prospectively with prior notice to the extent required by contract or law, and that disputed fees will be handled in accordance with the Dispute Resolution provision below.


Dispute, Refunds, and Late Payment

Disputes concerning assessed fees must be submitted in writing to the Service Provider within 60 days of the fee assessment date. The Service Provider will acknowledge receipt promptly and will investigate in a commercially reasonable manner. Failure to timely dispute a charge constitutes acceptance of the charge except as otherwise required by law.

Late payment on invoiced fees will incur a late fee of and interest at a rate of until paid, subject to applicable legal limitations.

Revocation and Amendment

Client may revoke this consent by providing written notice to the Service Provider. Revocation will become effective 30 days after receipt of such notice unless a shorter period is required by law. The Service Provider may amend the fee schedule prospectively upon providing written notice to Client as required by contract or law.
