Cancellation must be delivered at least seven (7) business days prior to the next scheduled debit to allow processing. Creditor reserves the right to terminate the debit arrangement with reasonable notice to Account Holder.

Disputes and Refunds: In the event of an unauthorized or disputed debit, Account Holder must notify both Creditor and their financial institution promptly. Creditor will investigate bona fide disputes and, where an error is established, will arrange a refund or adjustment in accordance with applicable clearing rules. Account Holder's right to dispute a debit is subject to applicable law and the bank's dispute procedures.

Changes to Account Details: Account Holder must advise Creditor in writing of any change to bank account details sufficiently in advance of the next scheduled debit to permit processing. Creditor is not liable for debits that fail due to inaccurate or outdated account details provided by Account Holder.

Liability and Indemnity: Account Holder indemnifies Creditor against any loss, liability, claim or expense arising from failure to provide accurate bank account details, from insufficient funds where scheduled debits are presented, or from any breach of this authorization by Account Holder. Creditor will make reasonable efforts to notify Account Holder of failed debits and may apply applicable fees for returned items as disclosed in Creditor's schedule of fees.

Privacy: Account Holder consents to Creditor collecting and using the information on this form for the purpose of administering direct debits, verifying account details, and communicating about payments. Creditor will handle personal and financial information in accordance with its privacy obligations and only disclose to service providers where necessary to effect debits or to comply with law.
