By submitting this Payment To Payee (PTP) request, the Requesting Organization represents and warrants that: (a) the sums requested are true, correct and due in accordance with the underlying agreement or goods/services rendered; (b) no portion of the payment has been previously paid unless disclosed; and (c) all withholding or tax obligations applicable to the payee have been addressed as required by law.

The Requesting Organization shall indemnify and hold harmless the Authorizing Organization and its agents from any claim, loss or liability arising from inaccurate information provided in this form, including but not limited to duplicate payments or incorrect beneficiary details, unless such loss results solely from the gross negligence or willful misconduct of the Authorizing Organization.
