What the Financial SecureAuth Order Form Is

The Financial SecureAuth Order Form is a standardized commercial document used to request, authorize, and record financial services or product orders that require identity-verified approval. It captures party details, order line items, payment terms, billing instructions, and authorization signatures. When completed electronically, the form must meet U.S. e‑signature legal tests (intent, consent, attribution, retention) under the ESIGN Act (15 U.S.C. ch. 96) and applicable state UETA or ESRA provisions to ensure enforceability.

Why this form matters for financial transactions

A clear, compliant order form reduces processing delays, supports auditability, and establishes the payer’s authorization and consent for charges or service delivery.
