Terms, Representations, and Authorization

By executing this Standing Instruction, the Client authorizes the financial institution identified above to debit the Debited Account for the amounts and on the schedule set forth in this form. This authorization constitutes a continuing instruction and shall remain in effect until the earlier of the Final Payment Date set forth herein or the institution's receipt of a written revocation delivered in accordance with the institution's procedures and accepted by the institution in its sole discretion.

The Client represents and warrants that the Client is the legal owner of the Debited Account or is legally authorized to give these instructions for the Debited Account, that all information supplied in this Standing Instruction is true and correct, and that sufficient funds will be available on each scheduled payment date. The Client agrees to indemnify and hold harmless the financial institution and the Payee for losses, claims, or expenses arising from incorrect account information or insufficient funds.

The financial institution will process payments in accordance with usual business practices; the institution shall not be liable for any interest, penalties, or other damages resulting from delay or failure to process a debit when such delay or failure results from causes beyond the institution's reasonable control, including but not limited to systems failure, acts of third parties, or improper instructions supplied by the Client.

The Client understands that any fees charged by the financial institution for processing standing instructions, returned items, or amendments are the Client's responsibility. The Client authorizes the institution to debit such fees from the Debited Account without further authorization.

Amendment or cancellation of this Standing Instruction requires written notice to the financial institution and must be received with sufficient time to permit the institution to act before the next scheduled debit. The institution may require a reasonable period to implement changes and may require additional documentation where applicable.

Data provided on this form will be used for the purposes of executing the Standing Instruction and administering related account activity. The Client authorizes the institution to disclose relevant information to the Payee or to service providers where reasonably necessary to effect payments.

What the Financial SI Template Is and when it’s used

The Financial SI Template is a standardized document used to collect structured financial statement information, entity identifiers, and signer authorizations for transactions, audits, or vendor onboarding. It groups key data—legal entity name, tax identification numbers, account details, effective dates, and signatures—into a consistent layout that supports automated review, compliance checks, and archival. Organizations use it to reduce manual entry, ensure consistent recordkeeping, and create a single source of truth for downstream processes such as payment setup, tax reporting, or KYC reviews.

Why a standardized Financial SI Template matters

A consistent template reduces processing time, lowers transcription error rates, and centralizes the evidence needed for audits or regulatory requests.

Why a standardized Financial SI Template matters

Typical users and stakeholders

Teams that prepare, review, or rely on financial disclosures commonly interact with the Financial SI Template.

  • Accounts payable and receivable teams managing vendor onboarding and invoice setup.
  • Compliance, audit, and risk teams validating tax IDs, certifications, and retention requirements.
  • Relationship managers and treasury staff who need signed authorizations for account access or payment instructions.

Use this template to standardize intake across departments and to create an auditable record of who provided what data and when.

Step-by-step: completing the Financial SI Template

Follow these logical steps to complete and validate the template before submission.

  • 01
Prepare Documents: Gather W-9, formation documents, bank records and any authorizations.

  • 03
Attach Evidence: Upload supporting attachments and reference them inline in the template.
  • 04
Sign and Certify: Obtain authorized signature(s), date entries, and retain the audit trail.

Typical online workflow configuration

Set up a digital workflow that places fields, assigns signers, and enforces authentication.

Field Configuration
Signature Field Assigned to signer; required
Date Field Auto-populate on signature
Attachment Field Optional; accept PDF/JPG/DOCX
Authentication Email + SMS or KBA as needed

How digital completion and eSubmission typically flow

A standard digital workflow moves the template from preparer to signer and into archival storage with an audit trail.

  • Upload: Uploader provides core template and attachments.
  • Place Fields: Add signature, date, and text fields for each recipient.
  • Send to Signer: Signer receives email or secure link to review and sign.
  • Store & Audit: Completed PDF and audit trail saved to records.

Delivery formats, integrations, and authentication options

Choose platform settings that match your security posture and destination systems.

  • Supported Formats: PDF, DOCX, XLSX
  • Integrations: Salesforce, NetSuite, Google Workspace
  • Authentication: Email, SMS, KBA

Configure the workflow to produce an unalterable signed PDF, preserve audit metadata, and push final copies to your document repository.

Essential elements of a professional Financial SI Template

Design the template so it enforces required data, supports calculations, and produces a complete audit record for compliance.

APIs & Integrations

Connectors for ERP and document stores enable automated posting and archival of completed templates.

Best practices for accurate and efficient completion

Follow these practices to lower rejection rates and speed processing.

Validate identity upfront
Verify signatory identity and authority before sending the template for signature to avoid rework.
Use conditional logic
Hide irrelevant fields for simplicity and to reduce user errors during completion.
Require attachments
Mandate essential supporting documents such as W-9 or formation certificates to prevent incomplete submissions.
Keep version control
Record template version and change history so reviewers know which iteration was executed.

Common preparation errors to avoid

  • Incomplete TIN or mismatched name causing backup withholding or verification delays.
  • Missing evidence of signer authority, leading to rejected or non-binding signatures.
  • Incorrect date formatting that affects effective date calculations and retention triggers.
  • Uploading unlabelled attachments that cannot be matched to the corresponding template fields.

Consequences of errors in financial templates

Tax Penalties: Late filing fines
Withholding Risk: 24% backup withholding
Contract Delays: Payment postponement
Audit Exposure: Increased regulatory scrutiny
