FINANCIAL SLIDING SCALE FEE AGREEMENT

This Financial Sliding Scale Fee Agreement ("Agreement") is made effective as of by and between the parties identified below. This Agreement sets forth the terms under which the Provider will charge fees for services based on the Client's demonstrated income and household size, subject to the eligibility, verification, payment, confidentiality, and termination provisions contained herein.

Payment is due in full as specified on the invoice or at the time of service unless a payment plan is agreed in writing. Late payments may be subject to a late fee not to exceed the lesser of 1.5% per month or the maximum allowable by law. Provider may suspend services for nonpayment in accordance with applicable law.

Provider will treat income documentation and financial information as confidential and will maintain reasonable safeguards. Disclosure of information is permitted where required by law or as necessary to verify income through third-party sources with the Client's authorization. Provider reserves the right to request documentation to verify income at any time. If verification reveals inaccurate information that resulted in an improper fee, Provider may adjust fees retroactively and invoice Client for any underpayment. Repeated misrepresentation constitutes material breach and may result in termination of services.

Termination and Remedies

Either party may terminate this Agreement pursuant to the Provider's termination policies or for material breach. Remedies for nonpayment include collection, suspension of services, and recovery of reasonable collection costs and attorneys' fees to the extent permitted by law.

Miscellaneous

This Agreement constitutes the entire agreement between the parties regarding the subject matter hereof and supersedes prior agreements. Amendments must be in writing and signed by both parties. If any provision is found unenforceable, the remaining provisions remain in effect. This Agreement shall be governed by the laws of the state identified below.
