By signing below, the Client authorizes the Receiving Bank and the Existing Account Bank to take the actions necessary to effect the switch described in this form. This includes sending instructions to creditors, cancelling or re‑routing active mandates, and accessing account records solely for the purpose of completing the switch. The Client certifies that they are authorized to make this request for all account holders named on the Existing Account.

The Client further authorizes the Existing Account Bank to disclose to the Receiving Bank such information as is reasonably required to execute the switch, including confirmation of account balances, standing instruction details, and pending transactions. The Receiving Bank and the Existing Account Bank shall be released from liability for actions taken in good faith pursuant to this authorization, except in the case of gross negligence or wilful misconduct.

The Client agrees to indemnify and hold harmless the Receiving Bank and the Existing Account Bank against claims, losses, liabilities, costs and expenses arising from incomplete, inaccurate, or late information provided by the Client or from any third‑party creditor that fails to accept the switch instruction.

This authorization remains in effect until the Receiving Bank has provided written confirmation that the switch has been completed or until the Client provides written revocation to both institutions. Revocation does not affect actions already taken in reliance on this form.

Governing Law: This form and the actions carried out pursuant to it shall be governed by the laws of the jurisdiction in which the Receiving Bank maintains the New Account, and the Client submits to the exclusive jurisdiction of the courts of that jurisdiction for any dispute arising from this form.
