What the Fingerprint Authorization Form Is

A Fingerprint Authorization Form is a written consent that allows an organization to collect, submit, and use an individual's fingerprints for background checks, identity verification, licensing, or employment screening. The form documents the purpose, the requesting party, the scope of fingerprint use, and that the signer consents to electronic or physical fingerprint capture. It may accompany fingerprint cards, digital captures, or electronic transmission instructions and should include signer identity fields, effective date, and any required privacy disclosures.

Why a Clear Authorization Matters

A standalone authorization documents consent, creates an audit trail, and clarifies permitted uses—reducing privacy risk and supporting compliance with federal and state rules including ESIGN and UETA.

Why a Clear Authorization Matters

Who Typically Completes This Form

Use varies by industry and jurisdiction; include any agency-specific wording required by the recipient to avoid processing delays.

  • Employers and HR teams performing pre-employment background checks or security clearances.
  • Licensing boards and professional regulators collecting fingerprints for credentialing and discipline checks.
  • Agencies and vendors conducting criminal history checks for volunteers, contractors, or employees.

How to Complete the Fingerprint Authorization Form

Follow these steps in order to ensure the form is complete, attributed, and acceptable for agency submission.

  • 01
Prepare ID: Gather government-issued photo ID to verify the signer in person or via digital credential analysis.
  • 02
Fill Contact Details: Enter legal name, DOB, and address exactly as on ID to match background check records.
  • 03
State Purpose: Specify the exact reason for fingerprinting and name the receiving agency or employer.
  • 04
Sign and Date: Sign in ink or complete eSignature fields; include printed name, title, and date in MM/DD/YYYY format.

Essential Elements to Include on a Professional Form

A complete Fingerprint Authorization Form combines identity fields, consent language, purpose description, retention terms, submission instructions, and signature blocks to ensure legal validity and smooth processing.

Identity Fields

Full legal name, aliases, date of birth, current and prior addresses, and government ID details ensure accurate matching in fingerprint searches.

Consent Clause

Clear language authorizing fingerprint collection and specifying permitted uses reduces ambiguity and supports ESIGN/UETA-compliant electronic consent.

Purpose Statement

A concise description of why fingerprints are collected (for employment, licensing, or security) and the recipient agency or entity.

Retention Notice

Specify how long fingerprints or results will be retained and reference applicable retention rules or policies.

Submission Instructions

Provide steps for physical card submission, digital capture formats, required agency codes, and any mailing or portal details.

Signature Block

Include signature, printed name, date, and a checkbox or initial confirming consent to electronic processing if eSubmitted.

Key Security and Compliance Data to Note

Encryption: TLS 1.2/1.3 in transit; AES-256 at rest
Audit Trail: Timestamp, IP, and action history retained
HIPAA: BAA required for PHI-containing submissions
21 CFR Part 11: Compliant when required for FDA-regulated records
Authentication: Support for MFA and KBA proofing
Certifications: SOC 2 Type II, ISO 27001, PCI DSS

Typical Submission Flow for Fingerprint Authorizations

Most workflows follow a predictable path from consent capture to fingerprint submission and final reporting; adapt steps to the receiving agency's requirements.

  • Capture Consent: Signer completes authorization form and signs physically or electronically.
  • Collect Prints: Fingerprints captured on an agency card or via a certified digital scanner.
  • Transmit Data: Submit prints and authorization to the designated agency or vendor channel.
  • Receive Results: Background report returned and linked to the authorization for audit purposes.

Configuring an Online Fingerprint Authorization Workflow

Configure form fields, signer authentication, and routing rules so digital submissions meet recipient requirements and create a defensible audit trail.

Field Configuration
Signer Authentication Email + SMS code or SSO
Required Fields Full name, DOB, address, ID details
Routing Auto-forward signed form to vendor and HR
Retention Setting Store signed copy for required retention period

Technical Considerations for eSubmission

Ensure the platform can export audit logs and signed PDFs compatible with agency ingestion and long-term recordkeeping.

  • File Formats: PDF, DOCX, and certified scan images
  • Integrations: Connectors for HRIS, NetSuite, or cloud storage
  • Authentication: SMS codes, KBA, or enterprise SSO

Typical Timelines and Processing Expectations

Processing times depend on the receiving agency, submission method, and any backlog; plan for variable turnaround and include expected dates on the form.

Submission Acknowledgement:

Expect confirmation within 24–72 hours for electronic submissions

Agency Processing:

Background checks commonly take 1–6 weeks depending on the agency

Expedited Requests:

Some jurisdictions offer expedited processing for additional fees

Record Updates:

Allow 1–2 business days for HR systems to reflect results

Dispute Resolution:

Plan 30–90 days if results are contested or require appeal

Common Mistakes That Delay Processing

  • Submitting mismatched names or dates of birth between form and ID, causing identity-match failures and rework with the agency.
  • Failing to include agency-specific codes or reason-for-request fields, resulting in rejected submissions or additional follow-up.
  • Using low-quality scans or improperly captured fingerprints, which leads to unreadable prints and reprints.
  • Not retaining signed consent or audit logs long enough to meet regulatory or agency requirements, complicating disputes.

Principal Risks and Potential Consequences

Invalid Consent: Form defects can render consent legally insufficient
Privacy Breach: Unauthorized disclosure may trigger state breach laws
Regulatory Penalty: Fines under HIPAA or state privacy statutes possible
Criminal Liability: False statements may expose signers to prosecution
Processing Delay: Incomplete forms delay hiring or licensing decisions
Rejection: Agencies can reject noncompliant submissions

Real-World Uses of Fingerprint Authorizations

These short examples show how typical organizations structure authorization forms to meet industry needs and agency rules.

Healthcare Screening

A hospital collects fingerprints for staff credentialing

  • Prints linked to provider ID and licensure checks
  • The form includes HIPAA privacy language and a BAA clause, and stores signed authorizations for six years to meet regulatory retention requirements.

Professional Licensing

A state board requires fingerprints for license applicants

  • Board code and purpose are specified on the form
  • Applicants provide government ID and consent; signed forms are transmitted electronically to the state vendor to expedite background checks for license issuance.
