What the Fitness Assessment Statement Is and When to Use It

A Fitness Assessment Statement documents an individual's physical or mental capability to perform specified duties, return to work, or participate in a program. Typically completed by a licensed clinician, occupational health provider, or authorized evaluator, it records findings, restrictions, recommended accommodations, and an effective date. Organizations use the statement to support reasonable-accommodation decisions, verify fitness for safety-sensitive roles, or maintain personnel medical records. When handled electronically, the form must meet e-signature and privacy legal tests to ensure enforceability and confidentiality.

Why a Clear Fitness Assessment Statement Matters

A concise, accurately completed statement reduces liability, documents medical fitness decisions, and creates a reproducible legal record for employers and clinicians. Proper completion supports ADA accommodation processes, workers' compensation outcomes, and occupational safety determinations.

Why a Clear Fitness Assessment Statement Matters

Who Commonly Prepares and Uses These Statements

Employers, occupational health clinicians, and program administrators rely on Fitness Assessment Statements to record fitness determinations and related restrictions.

  • Occupational Health Clinicians and Physicians who perform exams and record medical findings for safety-sensitive positions.
  • Human Resources and Employer Representatives who need documented evidence for ADA and return-to-work decisions.
  • School or Program Administrators who review fitness for participation, accommodations, and liability mitigation.

Use the completed statement as part of personnel records, accommodation files, or medical clearance attachments while observing privacy rules such as HIPAA.

Step-by-Step: Completing a Fitness Assessment Statement

Follow these sequential steps to collect, document, and archive the assessment correctly.

  • 01
Prepare form: Select the correct template and confirm employer requirements.
  • 02
Conduct exam: Perform clinical evaluation and record objective findings.
  • 03
Document findings: Enter functional limitations, restrictions, and recommended accommodations.
  • 04
Sign and store: Obtain signature, date the form, and retain per recordkeeping rules.

Typical Workflow for Electronic Completion and Routing

A standard eWorkflow minimizes friction while preserving legal evidence such as audit trails and retention copies.

  • Upload: Upload the prepared PDF or template to the signing platform.
  • Place fields: Assign signature, date, and text fields to the evaluator and requester.
  • Authenticate: Use signer authentication appropriate to risk and privacy requirements.
  • Archive: Store signed copy and audit trail in secure records with access controls.

Configuring an Online Workflow for the Statement

Set up the document and signer sequence to match clinical and employer approval steps.

Field Configuration
Signature Field Assign to clinician with required date field.
Text Blocks Lock labels, allow clinician edits only where appropriate.
Authentication Use email plus SMS code or ID verification for higher-risk cases.
Retention Setting Enable read-only archival and exportable audit trail.

Technical Considerations for eSubmission and Storage

Choose a platform that supports secure storage, audit trails, and required integrations for clinical workflows.

  • File formats: PDF and DOCX supported
  • Integrations: Works with EHR and HR systems
  • Authentication: Offers multi-factor options

Verify the platform provides HIPAA controls, audit logs, and exportable records to meet legal and organizational retention policies.
