Waiver and Release of Liability

In consideration of being permitted to participate in the fitness class or program, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge and covenant not to sue the instructor, studio, owners, operators, agents and employees (collectively the Released Parties) for any and all liability, claims, demands, or causes of action for personal injury, property damage, wrongful death, or other loss arising out of or related to my participation, whether caused by the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.

I further agree to indemnify, defend and hold harmless the Released Parties from and against any and all claims, liabilities, losses, damages, costs and expenses (including reasonable attorneys' fees) arising out of or related to my participation or breach of this Agreement.

Emergency Medical Treatment Authorization

If I am injured or become ill during participation and I am unable to consent, I authorize the Released Parties to secure necessary medical treatment and transportation. I agree that I am financially responsible for any medical or related expenses incurred as a result of such treatment.
