What the Flooring Sample Request Form Does

The Flooring Sample Request Form documents requests for physical flooring samples or swatches and standardizes how sellers, manufacturers, contractors, and designers collect delivery and product details. Typical fields include requester name, company, phone, delivery address, product SKU or color code, sample quantity, project notes, and billing or account authorization. The form supports fulfillment tracking, internal approvals, and returns handling, and can be executed on paper or electronically. When used online, include an audit trail and controls to meet U.S. electronic signature rules under ESIGN and state UETA laws where applicable.

Why a Formal Sample Request Matters

A standardized sample request reduces shipping errors, clarifies who pays or authorizes samples, and creates a record for inventory and cost tracking. It establishes expectations for delivery and return policies while enabling consistent fulfillment and reporting.

Product Details

SKU, color, finish, and sample ID to ensure the warehouse selects the exact items requested for evaluation.

Quantity and Size

Common Mistakes to Avoid

  • Incomplete delivery addresses or use of a P.O. box that the carrier cannot service, resulting in returned or delayed samples.
  • Omitting the exact SKU, color code, or manufacturer name and receiving incorrect or similar-looking samples instead of the requested item.
  • Failing to record authorization or account number up front, causing fulfillment to hold the request pending approval and delaying delivery.
  • Not capturing shipping carrier and tracking number in the form, which makes it difficult to confirm delivery or process returns.

Risks from Incorrect or Missing Information

Shipping Loss: Missing or wrong address
Inventory Cost: Untracked sample depletion
Chargebacks: Unauthorized billing disputes
Delay: Missing approvals
Noncompliance: PHI mishandling risk
Customer Impact: Project hold-ups
