The Organization acknowledges receipt of the donation when funds are received or when a binding pledge is accepted. The Organization will issue a written acknowledgment that includes the date and amount of the donation and a statement confirming whether any goods or services were provided in exchange for the contribution. If the Donor receives any goods or services in return, the Donor certifies the fair market value of such goods or services does not exceed the amount disclosed below.

The Donor understands that contributions are charitable gifts and, except for any stated goods or services, are generally irrevocable and non-refundable. The Organization will direct and expend funds consistent with the designated purpose to the extent lawful and feasible; if the designated purpose becomes impracticable, the Organization will use the funds for substantially similar charitable purposes as determined by the Organization in its reasonable discretion.

This Donation Form is effective as of the date of the Donor's signature below. Recurring authorizations remain in effect until the Donor provides written notice to terminate. Termination of a pledge does not relieve the Donor of payment obligations for installments that have already become due. Notice of termination or modification must be mailed or delivered to the Organization at the address set forth above or to the contact provided by the Organization.

Representations and Authorization

The Donor represents that the Donor has full power and authority to make this donation and that the funds are not subject to any lien, encumbrance or restriction inconsistent with the terms of this Donation Form. If paying by card or ACH, the Donor authorizes the Organization to charge the account as set forth above in accordance with the stated schedule.

Governing Law; Entire Agreement

This Donation Form shall be governed by the laws of the state indicated below without regard to conflict of laws principles. This document constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior understandings, whether written or oral.
