What a Gift Card Request Form Is and when it’s used

A Gift Card Request Form is a standardized record used by organizations to authorize purchase, issuance, or replacement of retail or prepaid gift cards for customers, employees, or partners. It documents requester identity, recipient details, card value, purpose, and approval workflow so finance and compliance teams can track expenses, reconcile balances, and meet audit or unclaimed-property requirements. The form can be paper or digital; when submitted electronically it should capture signer intent, consent to transact electronically, and a tamper-evident audit trail to preserve evidentiary value for future review.

Why keeping a formal request form matters

A formal Gift Card Request Form reduces reconciliation errors, maintains a clear audit trail for approvals and payouts, and supports compliance with internal controls and state unclaimed-property rules. It creates a single record used by finance, HR, and legal for reporting and dispute resolution.

Why keeping a formal request form matters

Who typically completes or approves gift card requests

Requesters and approvers vary by use: employee recognition, customer promotions, vendor incentives, or refunds; central teams control issuance.

  • HR and People Ops — Initiates employee recognition or referral awards, tracks taxable benefits.
  • Marketing Managers — Requests promotional cards for campaigns with campaign codes and budget center.
  • Customer Service — Authorizes refunds or compensations tied to specific ticket IDs.

Proper role separation — requester, approver, finance — reduces fraud risk and supports later reconciliation.
