What the Gift Donation Form Is and when it’s used

A Gift Donation Form documents a donor's transfer of money, goods, or property to a nonprofit, institution, or individual. It records donor identity, description and value of the gift, any restrictions, the date of transfer, and signatures. Organizations use the form to acknowledge contributions for internal records and tax substantiation. Donors use it to document charitable intent and to support deduction claims where applicable. The form can cover cash donations, in-kind gifts, stocks, and tangible personal property.

Why a formal Gift Donation Form matters

A clear, signed form protects both donor and recipient by documenting intent, conditional terms, delivery date, and acknowledgement for tax purposes under IRS rules. It reduces disputes and supports recordkeeping and audit readiness.
