What the Graffiti Removal Request Form Is

The Graffiti Removal Request Form is a standardized document used to request removal of graffiti from private or public property. It collects the property location, surface type, size estimate, date and time observed, photographic evidence, owner or manager authorization, preferred contractor or city service selection, and any insurance or cost-recovery instructions. The form also captures signatory consent and contact information for scheduling and liability purposes. When submitted electronically with a compliant eSignature platform such as signNow, the form preserves an audit trail and supports lawful electronic execution under ESIGN and state UETA statutes.

Why a Structured Request Form Matters

A Graffiti Removal Request Form documents location, authorization, and evidence so property owners or municipalities can schedule cleanup, assign responsibility, and pursue cost recovery. Using an auditable electronic form reduces processing time, creates a verifiable record, and helps meet municipal or contractual compliance requirements.

Why a Structured Request Form Matters

Who Typically Files This Form

Property owners, tenants, property managers, and municipal public works staff commonly use this form to report graffiti and request removal services.

  • Private property owners — document permission, track costs, and provide contact details for contractors.
  • Municipal agencies — collect reports, coordinate city crews, and maintain public records for enforcement.
  • Property managers and HOAs — centralize service requests, approve vendor access, and manage restoration budgets.

The completed form provides a clear chain of authorization and factual record useful for scheduling, invoicing, and any insurance or legal follow-up.

Essential Elements of a Professional Request

Key elements of a professional Graffiti Removal Request Form ensure authorization, accurate location data, evidence capture, cost allocation, scheduling details, and clear signatory responsibility.

Location

Enter the precise street address, unit or parcel number, GPS coordinates if available, and landmark details. Accurate location reduces response time and minimizes risk of misdirected cleanup work.

Evidence

Attach clear photographs showing the full extent of graffiti, with close-ups of identifying marks and a ruler or object for scale. Timestamped images support insurance claims and enforcement actions.

Authorization

Provide owner or authorized agent name, relationship to property, contact phone and email, and written consent to enter property for cleanup. Owner authorization avoids contractor liability disputes.

Scope & Cost

Describe surface type, paint medium, approximate square footage, and preferred removal method. If cost recovery is sought, state billing instructions and whether the city, owner, or insurer will be billed.

Scheduling

Include requested removal date, earliest available access times, and any site restrictions. Provide contact for on-site access and vendor arrival windows to prevent unnecessary return visits.

Signatures

Each responsible party must sign and date the form. For electronic submissions, include an auditable eSignature, signer identity method, and a printed name field for records.
