The Member authorizes the Facility to charge the payment method above in accordance with the schedule. Member agrees that failed payments may result in suspension or termination of membership and that the Member is responsible for collection costs and reasonable attorney fees for overdue amounts.

Renewal: Unless otherwise indicated, memberships will automatically renew in accordance with the payment schedule until terminated in writing.

Termination: Either party may terminate this Agreement for convenience by providing written notice to the other party at least days prior to the intended termination date. Facility may immediately suspend or terminate membership for safety violations, non-payment, or breach of this Agreement.

HEALTH, WAIVER AND RELEASE

The Member represents that they are physically able to participate in exercise and fitness activities. The Member agrees to follow all Facility rules and posted instructions. The Member acknowledges that use of the Facility involves inherent risks, and the Member hereby releases the Facility, its owners, officers, employees, and agents from liability for personal injury, property damage, or wrongful death arising from ordinary negligence to the fullest extent permitted by law.

Health Conditions: The Member affirms that they have disclosed any medical conditions or medications that may affect safe participation:

Emergency Treatment Consent: I consent to emergency medical treatment if, in the opinion of Facility personnel, such treatment is necessary.

CONFIDENTIALITY

Both parties agree that personal data and membership account information shall be treated as confidential and will not be disclosed except as required by law or to process payments, provide services, or where the Member has given consent. Facility will maintain reasonable administrative, technical, and physical safeguards to protect Member information.

GOVERNING LAW

This Agreement shall be governed by and construed in accordance with the laws of the state in which the Facility is located, without regard to conflict of law principles. Venue for any dispute arising out of this Agreement shall lie in the state or federal courts located in that jurisdiction.

ENTIRE AGREEMENT

This document, together with any addenda signed by the parties, constitutes the entire agreement between the Member and the Facility concerning membership, superseding all prior negotiations, representations, and agreements. Any modification must be in writing and signed by both parties.

MEMBER ACKNOWLEDGMENT
