Best Practices to Reduce Risk and Improve Data Quality

Follow these best practices to improve safety, data quality, and compliance when using the Guidebook.

Confirm clinician medical clearance and instructions
Require clinician review and written clearance for individuals with chronic conditions or on medications. Document specific instructions for medication holds or adjustments, and ensure the clinician signs and dates the Guidebook before the fast begins.
Standardize vital measurements and recording times
Use consistent devices and units for weight, blood pressure, and pulse. Record the exact time of each measurement to allow trend comparison. Note device type and posture to improve clinical interpretation of serial data.
Use clear consent language and withdrawal procedures
Explain the right to withdraw consent at any time, how to contact supervising clinicians, and procedures for emergency termination. Provide a paper copy on request and record any withdrawal or modification in the Guidebook with timestamps and signatures.
Preserve audit trail and version control
Keep signed copies with a certificate of completion, including timestamps, IP addresses, and signer authentication method. Use version numbering for edits and never overwrite original signed records; retain older versions for compliance and potential review.

Potential Consequences of Incomplete or Incorrect Records

Clinical Harm: Missed contraindications risk injury
Liability Exposure: Civil claims or malpractice risk
Insurance Denial: Coverage disputes or claim denial
Data Breach: HIPAA violations and fines
Research Noncompliance: Invalid data or review sanctions
Documentation Errors: Incorrect dates or names

Common Preparation and Documentation Pitfalls

  • Incomplete medication lists omit held or adjusted drugs, increasing risk and complicating clinician decisions during the fast.
  • Using inconsistent measurement units or times undermines trend analysis and may mask adverse vital sign changes.
  • Failing to document consent withdrawal, emergency contacts, or transport plans causes delays during incidents and legal disputes.
  • Relying on unsigned or image-only signatures without an audit trail reduces enforceability and complicates audits.
