What the Health Information Statement Is and When It Applies

A Health Information Statement is a written record used to collect a patient's medical and contact details, authorization for release or use of protected health information (PHI), and attestations about current health status or prior treatments. Providers, insurers, researchers, and schools commonly use this document to document consent, share records, and support clinical decision-making. The statement may include demographics, medical history, medications, emergency contacts, insurance coverage, and specific release instructions. Because it often contains PHI, handling must align with applicable privacy laws and organizational policies.

Why a Clear Health Information Statement Matters

A well‑prepared Health Information Statement reduces clinical risk, speeds administrative processing, and creates a clear legal record of patient consent and disclosures. Properly completed statements support HIPAA compliance (45 CFR §164.502) and, when signed electronically, meet ESIGN and UETA standards for enforceability.

Why a Clear Health Information Statement Matters

Who Prepares and Who Signs a Health Information Statement

Typical creators include clinical staff, medical records teams, insurance coordinators, and institutional administrators who need accurate patient data for care or billing.

  • Patients and legal guardians sign to consent and attest to accuracy; guardians sign for minors or incapacitated adults.
  • Health care providers and intake staff complete clinical sections and verify identifiers against medical records.
  • Insurance administrators and release-of-information personnel use the form to authorize disclosure and route records to payers.

The completed statement becomes part of the legal medical record and must be stored, disclosed, and retained according to privacy and records rules.

Essential Parts of a Professional Health Information Statement

A concise, standardized statement helps staff collect consistent data and reduces the need for follow-up. Include clear headings, help text, and required-field markers to speed processing.

Patient Identifiers

Full legal name, DOB, contact, and a government ID or medical record number to prevent mismatches and ensure correct chart assignment.

Medical History

Brief problem list, relevant diagnoses, allergies, current medications, and chronic conditions so clinicians can triage and treat safely.

Authorization Scope

Clear description of which records may be released, to whom, and for what purpose, including specific date ranges or categories of information.

Emergency Contacts

Names, relationships, and phone numbers for rapid contact in urgent situations and to confirm decision‑maker identity when needed.

Insurance Details

Payer name, policy or subscriber number, and billing contact information to support claims and financial coordination.

Signature and Attestation

Signed date, printed name, relationship to patient, and witness or notary fields if required by state or institutional policy.
