I, the undersigned patient or authorized representative, authorize sampling of suspected asbestos-containing bulk material described above and submission of the sample(s) to a qualified laboratory for analysis. I understand that sample collection may involve disturbance of building materials and that the sampling team will use established containment and protective measures to minimize release.

I understand that laboratory analysis will report the presence or absence of asbestos fibers and relevant analytical findings. Test results will be maintained in the patient\'s medical record and may be released to the treating physician, occupational health personnel, and others as necessary for treatment, public health reporting, or as required by law. I expressly authorize release of laboratory results and related sample documentation for these purposes.

I acknowledge that I have the right to withdraw this authorization at any time prior to sampling by providing written notice to the facility; however, withdrawal will not apply to information or samples already released or processed. This authorization will expire on the date indicated below unless revoked earlier in writing.

Privacy Notice: Protected health information created or collected in connection with this sampling and testing will be treated confidentially in accordance with applicable privacy laws and facility policy. By signing below I acknowledge receipt of the facility\'s privacy practices regarding use and disclosure of health information in connection with environmental exposure evaluation.

By signing, I certify that the information provided on this form is true and complete to the best of my knowledge. I understand the purpose of the sampling and authorize the collection, analysis, and release of results as described above. I further acknowledge that sampling and analytical results do not constitute a comprehensive environmental survey of the entire facility unless otherwise stated in accompanying documentation.
