Additional Instructions and Limitations

The Patient may impose written limitations on the authority of any Authorized Signatory. Absent explicit written limitation, the facility and its personnel may rely on the representation of an Authorized Signatory acting within the scope of authority granted herein. The facility is not required to take any action that would violate law, professional standards, or the Patient’s best interests.

Revocation and Expiration

This authorization remains effective until the expiration date(s) specified above for each Authorized Signatory, or until earlier revoked by the Patient in writing. Revocation takes effect when the healthcare provider receives written notice, except to the extent that the provider has already acted in reliance on this authorization. Any attempt to revoke verbally should be confirmed in writing.

HIPAA Acknowledgment and Consent to Disclose

By signing below, the Patient acknowledges that the Patient has been informed of the facility’s privacy practices and authorizes disclosure of protected health information to the persons identified in this form to the extent necessary to carry out the functions described. This authorization complies with the Health Insurance Portability and Accountability Act and related privacy regulations to permit disclosure to the Authorized Signatories named above.
