Specific services to be provided under this Agreement are described below:

Fees, Payment and Patient Responsibility

The Patient authorizes the Billing Entity to retain billing and collections fees as follows. Billing Entity Fee (percentage of collected insurance payments): Patient co-pay / co-insurance responsibility: .

The Patient remains ultimately responsible for all charges not paid by insurance. Billing Entity will apply insurance payments to outstanding charges, deduct its agreed fee, and bill the Patient for any remaining balance. The Patient agrees to pay billed balances within days of statement receipt.

Assignment, Authorization and Release

By signing this Agreement, the Patient assigns to the Billing Entity the right to receive insurance payments and authorizes payment of insurance benefits directly to the Billing Entity for services rendered. The Patient authorizes any health plan, employer, or other payor to release to the Billing Entity information necessary to process claims and to resolve claims disputes.

Statements, Electronic Communications and Notices

The Patient consents to receive billing statements, explanations of benefits summaries, and collection notices via electronic means if an email is provided. Electronic delivery will be considered effective upon transmission.

Collections, Interest and Legal Costs

Unpaid balances may be referred to collections. The Patient agrees that past-due balances may incur interest at a rate not to exceed the maximum lawful rate and that the Billing Entity may recover reasonable collection costs, including attorney fees, court costs and collection agency fees where permitted by law.

HIPAA Authorization and Privacy Acknowledgment

The Patient acknowledges receipt of the Billing Entity's privacy practices related to protected health information used for billing and payment purposes. The Patient authorizes the use and disclosure of protected health information to the extent necessary to prepare, submit, and appeal claims, and to coordinate care as related to billing.

Term, Termination and Miscellaneous

This Agreement becomes effective on the date signed by the Patient and remains in effect until revoked in writing. Either party may terminate this Agreement upon thirty (30) days' written notice; termination does not affect the Billing Entity's right to collect fees for work performed prior to termination.

This Agreement constitutes the entire understanding between the parties regarding billing services and supersedes prior agreements. Amendments must be in writing and signed by both parties. If any provision is held invalid, remaining provisions remain enforceable.

Dispute Resolution and Governing Law

Any dispute arising under this Agreement shall be resolved first through good faith negotiation. If unresolved, disputes shall be submitted to binding arbitration under the laws of: and judgment upon the arbitration award may be entered in any court of competent jurisdiction.
