HEALTHCARE BOOTCAMP AGREEMENT

This Healthcare Bootcamp Agreement ("Agreement") is entered into as of by and between Provider Name: (hereinafter "Provider") and Participant Name: .

PROGRAM DESCRIPTION

Provider will deliver a structured healthcare bootcamp program consisting of clinical assessment, supervised exercise sessions, nutritional guidance, and group health education (the "Program"). The Program will commence on and end on , unless earlier terminated in accordance with this Agreement.

MEDICAL CLEARANCE

Participant represents and warrants that they have consulted with a licensed healthcare provider as appropriate and are cleared to participate in the Program. Participant agrees to notify Provider promptly of any change in medical condition. Participant initials to confirm medical clearance:

ASSUMPTION OF RISK; RELEASE AND LIMITATION OF LIABILITY

Participant acknowledges that participation in fitness and clinical activities involves risk of injury, illness, or aggravation of existing conditions. Participant voluntarily assumes all risks and, to the fullest extent permitted by law, releases and holds harmless Provider, its employees, contractors, and agents from any and all claims, liabilities, losses, or damages arising from participation in the Program, except to the extent caused by Provider's gross negligence or willful misconduct.

Participant expressly agrees that Provider's liability for any claim under this Agreement shall be limited to direct damages not to exceed the fees paid by Participant to Provider for the Program during the twelve (12) months preceding the event giving rise to the claim.

PAYMENT, CANCELLATION, AND REFUND

Cancellation by Participant more than 14 days prior to program start will receive a refund of fees paid less administrative charges. No refunds will be provided for withdrawals after Program commencement except as required by law or at Provider's sole discretion. Provider reserves the right to cancel or reschedule the Program; in such event Participant's sole remedy is a full refund of amounts paid.

CONFIDENTIALITY; HIPAA AUTHORIZATION

Provider will maintain Participant health information in accordance with applicable privacy laws. Participant hereby authorizes Provider to use and disclose protected health information as reasonably necessary for treatment, billing, program administration, and emergency care. This authorization expires on:

EMERGENCY MEDICAL AUTHORIZATION

In the event of a medical emergency, Participant authorizes Provider to obtain medical treatment on Participant's behalf and to disclose necessary health information to emergency responders and treating facilities. Participant authorizes Provider to bill Participant's insurance for emergency care when appropriate.

PARTICIPANT RESPONSIBILITIES

Participant agrees to comply with Program rules, to follow individualized guidance provided by clinical staff, to disclose any changes in health status, and to refrain from conduct that endangers others. Participant acknowledges that noncompliance may result in removal from the Program without refund.

TERMINATION; INDEMNIFICATION

Provider may terminate Participant's enrollment for material breach, failure to follow medical guidance, or conduct detrimental to Program safety. Participant agrees to indemnify, defend, and hold Provider harmless from any third-party claims arising from Participant's negligence or willful misconduct.

GOVERNING LAW; SEVERABILITY

This Agreement shall be governed by the laws of the state of without regard to its conflicts of law principles. If any provision is found unenforceable, the remaining provisions will remain in full force and effect.

ENTIRE AGREEMENT

This Agreement, together with any program-specific addenda or schedules signed by the parties, constitutes the entire agreement between Participant and Provider regarding the Program and supersedes all prior agreements and understandings.
