Provider will maintain the confidentiality of Client's protected health information in accordance with applicable privacy laws. Provider's use and disclosure of Client information will be limited to purposes necessary for treatment, payment, and healthcare operations unless Client provides written authorization otherwise.

Client has been informed of the anticipated benefits and material risks associated with the services and may withdraw consent at any time by giving notice to Provider as set forth in this Agreement.

11. Limitation of Liability and Indemnification

To the fullest extent permitted by law, Provider's liability for damages hereunder shall be limited to direct compensatory damages and shall not include incidental, consequential, or punitive damages. This Agreement shall be governed by the laws of the state where services are provided. The parties agree to attempt good-faith negotiation of disputes and, if unresolved, to submit disputes to mediation prior to filing litigation. Each party bears its own costs unless otherwise ordered.
