Provider shall deliver professional healthcare services as agreed for the purposes described below. Services provided may include assessment, diagnosis, treatment planning and direct treatment. The Client acknowledges that the exact nature, frequency and duration of services will be determined by the Provider in consultation with the Client.

Fees, Billing and Payment

Unless otherwise agreed in writing, Client is responsible for all charges for services rendered. Provider will bill insurance as a courtesy if insurance information is provided; however Client remains ultimately responsible for fees not covered by insurance, including co-payments, co-insurance, deductibles and services denied by the insurer.

Cancellation and No-Show Policy

Client agrees to provide at least 24 hours' notice for cancellation of scheduled appointments. Provider may charge a cancellation or no-show fee where applicable. Repeated cancellations or missed appointments may result in termination of services.

Confidentiality and HIPAA Acknowledgment

Provider maintains records in accordance with applicable privacy laws. Client's protected health information will be disclosed only as permitted by law or with Client's written authorization. Client acknowledges receipt of the Provider's Notice of Privacy Practices and understands that the Notice describes how health information may be used and disclosed and how Client can obtain access to this information.

Authorization to Release Information

Client authorizes Provider to release and exchange medical information where necessary for treatment, payment, or healthcare operations, and to coordinate care with other health professionals or payers as needed. This authorization includes release of records to insurers, consultants, or other providers directly involved in Client's care.

Consent to Treatment

Client consents to routine diagnostic procedures and treatments recommended by Provider. Client acknowledges that no guarantees have been made regarding outcomes. Client has the right to ask questions, to refuse any treatment, and to withdraw consent at any time except where withdrawal would jeopardize Client's health in an emergency.

Termination; Governing Law

Either party may terminate this Agreement with written notice. Termination does not relieve Client of financial obligations for services rendered prior to termination. This Agreement will be governed by the laws of the state in which Provider maintains the primary place of business. Any dispute arising under this Agreement will be resolved in accordance with the Provider's dispute resolution procedures, subject to applicable law.
