By initialing below, Client authorizes Provider to submit claims and receive payment from Client's insurance carrier and assigns to Provider any insurance benefits payable for the services rendered. This assignment is subject to the terms and conditions of Client's insurance policy.

Consent to Treatment and Rights

Client hereby consents to the provision of healthcare services by Provider. Client acknowledges that Provider has explained the nature and purpose of the proposed services, potential material risks and benefits, reasonable alternatives, and the likely consequences of refusing treatment. Client has the right to ask questions and to withdraw consent at any time, subject to reasonable notice for scheduled services.

Authorization for Release of Medical Information

Client authorizes Provider to release protected health information to payers, referring providers, or as otherwise required for treatment, payment, or healthcare operations. Client further authorizes release of records to third parties as necessary for continuity of care or as authorized in writing by Client.

Privacy and HIPAA Acknowledgment

Provider maintains policies consistent with applicable privacy laws governing protected health information. Client acknowledges receipt of Provider's Notice of Privacy Practices and understands how health information may be used and disclosed. Client may request restrictions or confidential communications; such requests will be reasonably accommodated when feasible.

Term, Termination and Cancellation

This Agreement continues until terminated by either party upon written notice. Provider may terminate services for non-payment, failure to comply with treatment recommendations, or behavior that jeopardizes safety. Client remains responsible for fees incurred prior to termination, including reasonable costs associated with cancellation or early termination.

Liability, Indemnification and Insurance

Provider maintains professional liability insurance. To the fullest extent permitted by law, each party agrees to indemnify and hold harmless the other from claims arising from that party's negligence or willful misconduct. Provider's liability for damages arising out of professional services is limited to direct damages and shall not include punitive or consequential damages, except as required by law.

Dispute Resolution and Governing Law

The parties agree to first attempt to resolve disputes through good faith negotiation. If unresolved, disputes shall be resolved by binding arbitration conducted by a neutral arbitrator in accordance with applicable arbitration rules. This Agreement is governed by the laws of the state governing the Provider's principal place of business.

Miscellaneous Provisions

This Agreement constitutes the entire agreement between the parties regarding the subject matter and supersedes prior discussions and agreements. Amendments must be in writing and signed by both parties. If any provision is held invalid, the remaining provisions remain in effect. No waiver of any provision is effective unless in writing and signed by the party granting the waiver.
