Coach Responsibilities: The coach will provide structured health coaching focused on goal-setting, behavior change strategies, progress tracking, and resource referral as clinically appropriate. The coach will not provide medical diagnoses, prescribe medications, or deliver psychotherapy unless separately licensed and contracted to do so.

Confidentiality, HIPAA & Limits of Privacy

All personal health information and session notes will be treated as confidential and maintained in accordance with applicable privacy laws and the coach's privacy practices. The client acknowledges exceptions to confidentiality, including mandatory reporting obligations, threats of harm to self or others, suspected abuse of a vulnerable person, court order, or other disclosures required by law.

If the client experiences a medical emergency or is at imminent risk of harm, the client agrees to seek emergency services immediately. The coach will assist in connecting to urgent care resources if available but is not responsible for emergency response.

Right to Withdraw: The client may discontinue coaching at any time. The coach reserves the right to suspend or terminate services if the coach determines that coaching is no longer appropriate or if terms of this plan are breached.

Fees, Billing and Cancellation

Cancellation and Missed Sessions: Cancellations with less than 24 hours' notice may incur the full session fee. Repeated cancellations or no-shows may result in termination of services. The client agrees to pay for services rendered and to satisfy any outstanding balances.

Outcomes, Records & Termination

Progress Measurement: The coach and client will set measurable indicators. Progress notes will be retained in the coach's record. Upon termination, the client may request a summary of progress; the coach will provide a summary consistent with privacy practices and professional obligations.

Termination: Either party may terminate this plan with written notice. The coach may terminate for nonpayment, unsafe behavior, or demonstration that goals are not appropriate for coaching.
