Patient shall provide complete and accurate medical history, comply with prescribed treatment plans, attend scheduled appointments, and make timely payments for services. Failure to follow prescribed care or to provide accurate information may affect treatment efficacy and liability.

Provider Obligations

Provider shall deliver services in accordance with prevailing standards of care and applicable laws, maintain clinical records, and communicate treatment options, risks, and alternatives to Patient in a timely manner.

Consent for Treatment; Risks and Benefits

Patient acknowledges receipt of information concerning the nature, expected benefits, and material risks of the proposed treatment. Patient understands that no guarantee of results has been made and may withdraw consent at any time subject to reasonable notice and clinical considerations.

Confidentiality and HIPAA Authorization

Provider will maintain the confidentiality of Patient's protected health information consistent with applicable privacy laws. By signing this Agreement, Patient authorizes Provider to use and disclose protected health information for treatment, payment and healthcare operations as permitted by law.

Indemnification and Limitation of Liability

Each party shall indemnify, defend and hold harmless the other from claims arising from its own negligent acts or omissions. Except for liability arising from gross negligence or willful misconduct, Provider's aggregate liability for any claim arising under this Agreement shall not exceed the total fees paid by Patient to Provider for the specific services that gave rise to the claim.

Dispute Resolution and Governing Law

This Agreement shall be governed by the laws of the jurisdiction where Provider maintains primary practice. Parties agree to attempt to resolve disputes in good faith. If unresolved, disputes shall be submitted to binding arbitration unless both parties agree in writing to litigate.

Each party represents that it has the full authority to enter into this Agreement, that all information provided to the other party is true and correct to the best of its knowledge, and that it will comply with applicable laws and regulations in the performance of its obligations.
