Cancellation Policy: Client agrees to provide at least 24 hours advance notice for cancellations. Cancellations with less than 24 hours notice or missed sessions may be charged the full session fee unless waived for emergency circumstances.

Client agrees to pay fees in full at the time of service unless other arrangements are made. Counsellor may bill the insurance provided by Client if Client authorizes assignment of benefits below.

5. Confidentiality and Limits

All communications between Client and Counsellor are confidential and protected by applicable privacy laws and professional ethics. Client understands the following legally required exceptions to confidentiality: (a) reasonable suspicion of abuse or neglect of a child, elderly person, or dependent adult; (b) imminent risk of serious harm to self or others, including situations where the counsellor must take steps to protect potential victims; (c) information subject to a valid court order or subpoena; and (d) necessary disclosure for referral, consultation, or billing as authorized by Client.

Counsellor will make reasonable efforts to contact family or emergency services if Client is judged to be at imminent risk. Counsellor may consult with other qualified professionals about Client's care while keeping identifying information to a minimum.

6. Telehealth and Electronic Communications

Telehealth: Counselling via videoconference or telephone is available. By checking consent below, Client acknowledges understanding of the differences between in-person and telehealth care, including potential risks such as technology failure, privacy limitations, and delayed response in emergencies.

7. Risks, Benefits & Alternatives

Counselling can produce benefits such as reduced distress and improved functioning. There are no guarantees. Participation may also involve uncomfortable emotions, memory recall, or temporary increases in distress. Client has the right to discuss treatment alternatives with Counsellor and to withdraw consent at any time in accordance with Section 9 below.

8. Records, Release and Authorization

Counsellor maintains clinical records. Client authorizes release of information necessary for treatment coordination or billing as indicated below. Client may revoke authorization in writing, except where Counsellor has already acted on the authorization.

9. Termination and Withdrawal

Either party may terminate counselling at any time. Client is responsible for payment for services rendered through the date of termination. If Counsellor determines that continued services are not appropriate, Counsellor will provide referrals and a transition plan when feasible.

10. Miscellaneous

Governing Law: This Agreement shall be governed by the laws of the jurisdiction in which Counsellor practices. Amendments must be in writing and signed by both parties. If any provision is found invalid, other provisions will remain in effect.
