The Client/Guardian authorizes the Provider to secure emergency medical treatment for the Participant if necessary. The Client hereby authorizes Provider personnel to consent to emergency medical care if the Client or designated contact cannot be reached.

The Provider will maintain confidentiality of protected health information in accordance with applicable law. Termination, Suspension & Modification

Either party may terminate this Agreement with fourteen (14) days' written notice. The Provider may immediately suspend services when the Participant's behavior or medical needs present an imminent risk to safety, or for nonpayment, subject to written notice where reasonable. Modifications to services or schedule must be documented in writing and signed by both parties to be effective.

Liability, Indemnification & Insurance

The Provider will exercise reasonable care in the provision of services. To the fullest extent permitted by law, the Client agrees to indemnify and hold harmless the Provider and its staff from claims arising from the Client's actions or omissions. The Provider maintains general liability insurance; this Agreement does not waive any rights to recovery for gross negligence or willful misconduct.

Incident Reporting & Quality Assurance

The Provider will document and report incidents in compliance with applicable standards. The Client consents to reasonable monitoring, review, and quality assurance activities related to the services provided under this Agreement.

Authorization Duration & Revocation
