The patient hereby authorizes Provider to perform the above-named dental procedure(s). The patient acknowledges that Provider has explained the nature of the proposed treatment, expected benefits, foreseeable risks and complications (including, but not limited to, infection, nerve injury, tooth fracture, and need for additional procedures), and reasonable alternative treatments including no treatment. The patient understands that results cannot be guaranteed.

The patient has the right to ask questions and to withdraw consent at any time prior to the start of treatment. If local anesthesia, sedatives, or other medications are administered, the patient consents to such medications and acknowledges discussion of associated risks.

Financial Agreement

The patient is financially responsible for all fees for dental services rendered. Provider will file claims to assigned insurance on the patient's behalf when assignment of benefits is accepted, but the patient remains ultimately responsible for payment of charges not covered by insurance, including co-payments, deductibles, and non-covered services.

Assignment of Benefits: By signing this Agreement, the patient authorizes payment of insurance benefits directly to Provider to the extent permitted by law.

Legal Terms
