I authorize the dental providers and staff to perform diagnostic procedures and dental treatment as may be deemed necessary. I understand that the practice will explain the recommended procedures, risks, alternatives and expected outcomes. I acknowledge that no guarantees have been made concerning the results of any procedures or treatments.

Patient or responsible party is responsible for all charges for dental services. Insurance is considered a method of payment; any estimate of benefits is not a guarantee of payment. I authorize payment of dental benefits to the provider and assign benefits to the treating dentist to be applied to services rendered. I understand I am responsible for deductibles, co-payments and charges not covered by my carrier. If account is referred for collection, I agree to pay collection costs and reasonable attorney fees as permitted by law.
