I authorize the provider to perform diagnostic, preventive, restorative, prosthetic, surgical, optical and related procedures as may be deemed necessary or advisable for my dental and/or vision care. Procedures may include but are not limited to examinations, radiographs, prophylaxis (cleaning), fillings, extractions, endodontic treatment, periodontal therapy, lens fitting, refraction, prescription of corrective lenses or contact lenses, and temporary or permanent restorations.

I understand that all procedures involve risks including, without limitation, infection, bleeding, swelling, allergic reaction to medications or materials, nerve injury, failure of restorations, change in vision, adaptation difficulties with new lenses, and the possibility that treatment may not achieve desired results. I acknowledge that the nature and purpose of proposed treatments, reasonable alternatives and associated risks have been explained to me to my satisfaction.

I understand I have the right to ask questions and to withdraw consent at any time prior to treatment. Withdrawal of consent will not negate the provider's right to provide emergency care if necessary. If I am signing as a guardian or agent, I certify that I have the legal authority to consent on behalf of the patient.
