To the extent permitted by law, Patient agrees that Dental Practice shall not be liable for consequential, incidental or special damages. Patient agrees to indemnify and hold harmless the Dental Practice, its employees and agents from claims arising from Patient's failure to disclose material medical information or from Patient's noncompliance with instructions.

Termination; Governing Law; Dispute Resolution

This Agreement may be terminated by either party upon written notice. All disputes arising under or relating to this Agreement shall be resolved first by good faith negotiation.
