Recipient represents that it is duly organized and authorized to accept donations and that it will comply with all applicable federal, state, and local laws and regulations applicable to the receipt, storage, handling, and use of the Donation, including applicable privacy and health information protection requirements.

Recipient affirms it will maintain appropriate safeguards to protect patient privacy and health information and will not use the Donation in a manner that violates privacy obligations.

Tax Treatment and Acknowledgment

Donor acknowledges that Recipient may provide a written acknowledgement of receipt that states the nature and value of the Donation as required by applicable tax rules, but Recipient does not provide tax advice and makes no representation concerning the tax deductibility of any donation. Donor shall seek independent tax advice regarding deductibility and reporting.
