SCOPE OF DOULA SERVICES

CANCELLATION, RESCHEDULING & TERMINATION

The Doula reserves the right to terminate this Agreement for safety, breach, or if the Doula is unable to perform services due to illness or emergency; in such event payments for services not yet rendered will be prorated and refunded, less the earned retainer where applicable.

CONFIDENTIALITY & HIPAA AUTHORIZATION

Doula will maintain confidentiality of Client's health information, except as required by law or as authorized below. This authorization is voluntary and may be revoked in writing at any time except to the extent action has already been taken.

The Doula's liability for any claim arising under this Agreement is limited to direct damages up to the total fees paid by Client under this Agreement. Client agrees to indemnify and hold the Doula harmless from claims arising out of Client's failure to disclose relevant medical information or to follow the advice of licensed medical personnel.

EMERGENCY PROCEDURES

In the event of an emergency, the Doula will facilitate timely transfer of care to the appropriate medical facility and contact emergency services as necessary. Client authorizes the Doula to act to secure urgent medical assistance if Client is unable to do so.
