Procedure to be performed: Standard 12-lead electrocardiogram (ECG) with technician interpretation and physician final interpretation. The ECG records electrical activity of the heart and assists diagnosis of arrhythmia, ischemia, conduction abnormalities, and other cardiac conditions.

Risks and limitations: The ECG is noninvasive. Potential minor risks include skin irritation from electrodes and transient discomfort. An ECG provides a momentary electrical tracing and may not detect intermittent cardiac events. It does not substitute for extended monitoring when indicated.

Right to refuse or withdraw: I understand that I may refuse or withdraw consent at any time prior to the procedure without affecting my right to future care or treatment.

I consent to performance of the ECG and authorize release of ECG results to my treating providers for clinical care. I certify the information provided on this form is true and accurate to the best of my knowledge.
