This Healthcare Enrollment Agreement ("Agreement") sets forth the terms under which the undersigned Patient enrolls for medical and related services with the Provider. Patient Name: . Enrollment Effective Date: .

Billing, Payment and Assignment of Benefits

Patient is responsible for payment of co-payments, deductibles, co-insurance and services not covered by insurance. Patient authorizes Provider to bill the listed insurer and to receive payment directly from the insurer. Patient agrees to reimburse Provider for any portion of billed services denied by an insurer due to patient or subscriber noncompliance.

Cancellation, Termination and Amendments

Either party may terminate enrollment upon written notice. Provider may terminate or modify services for nonpayment, material breach, or where clinically necessary. Amendments to this Agreement must be in writing and signed by Patient and an authorized representative of Provider.

Notices and Legal Provisions

Notices required under this Agreement shall be in writing and delivered to the addresses on file. This Agreement shall be governed by the laws of the state where the Provider maintains its principal place of business. If any provision is held invalid, the remaining provisions remain enforceable.

By signing below, Patient certifies that the information provided is true and complete to the best of their knowledge, that they have read and understand this Agreement, and that they authorize Provider to render care and bill insurance as indicated.
