Payment is due within days of statement unless otherwise agreed in writing. Patient agrees to pay charges not covered by insurance, including deductibles, co-payments, co-insurance, and non-covered items.

By signing this Agreement the patient (or responsible party) acknowledges financial responsibility for all services rendered by the Provider, regardless of insurance benefits. The patient authorizes assignment of benefits and requests payment be made directly to Provider where allowed by insurer.

BILLING, LATE FEES & COLLECTIONS

Statements will be issued to the address on file. Past‑due balances may be subject to a late fee of or the maximum allowed by law. Account balances referred to a collection agency or attorney will incur additional collection and legal costs, including reasonable attorney fees, which the patient agrees to pay.

AUTHORIZATION TO RELEASE INFORMATION

I authorize Provider to use and disclose medical information necessary to process claims and obtain payment from my insurer, workers' compensation carrier, or other responsible third parties. This authorization includes the release of records required for benefits determination and appeals.

This authorization will remain in effect until unless revoked earlier in writing. Revocation does not affect prior actions taken in reliance on this authorization.

HIPAA AND COMMUNICATION CONSENT

I acknowledge receipt of the Provider’s Notice of Privacy Practices and consent to communications regarding my appointments, billing, and clinical matters at the contact information on file. I consent to leave voice messages and send electronic messages, unless I have restricted such communications in writing.

DISPUTE RESOLUTION & GOVERNING LAW

Except where prohibited by law, any dispute arising under this Agreement shall be resolved by binding arbitration with a single arbitrator under the rules agreed to by the parties. This Agreement shall be governed by the laws of the state in which the Provider maintains the principal place of business.

MISCELLANEOUS

The patient certifies that the information provided is true and correct to the best of their knowledge. The patient understands that insurance coverage determinations are the responsibility of the insurer and that Provider may bill the patient directly for any portion not paid by the insurer. Any modification to this Agreement must be in writing and signed by both the patient and an authorized representative of the Provider.
