Payment Methods, Returned Items & Collections

Accepted payment methods include cash, check, debit or credit card. Payment arrangements or a payment plan may be requested and must be documented in writing. If a check is returned for insufficient funds, a returned-check fee will be assessed in addition to any bank charges.

Delinquent Accounts and Collections: Accounts not paid within the stated payment terms may be subject to collection action, reporting to credit agencies, and recovery of collection costs, including reasonable attorney fees and court costs if applicable.

Cancellation, No-Show & Missed Appointment Policy

Cancellation: Appointments cancelled with less than 24 hours' notice or missed appointments may be charged a cancellation/no-show fee. Fees for missed appointments are separate from clinical charges and are the responsibility of the patient.

Estimates: Any estimate of charges is approximate and not a guarantee of coverage or payment by any third-party. The patient is responsible for charges not covered by insurance.

Billing Disputes: To dispute a charge, patient must notify the provider in writing within 30 days of the statement date. The provider will review and respond; undisputed portions remain payable in accordance with this agreement.

Privacy and Acknowledgments

HIPAA Acknowledgment: I acknowledge that I have received or been offered the provider's Notice of Privacy Practices describing how medical information about me may be used and disclosed, and my rights with respect to that information.

Governing Law: This agreement is governed by the laws of the state where services are rendered. Any dispute arising under this agreement shall be resolved in the state or federal courts located in the jurisdiction of the provider's principal office where services were provided, unless the parties mutually agree to alternative dispute resolution.
