The undersigned acknowledges and agrees that the Provider's fees for services rendered are separate from insurance benefits. The Provider will bill the identified insurer as a courtesy when valid insurance information is provided; however the patient remains ultimately responsible for all charges. Estimated fees below are for planning purposes only and do not constitute a guarantee of coverage or final charge.

Payment is due in accordance with Provider policy. Co-payments, co-insurance, and deductibles are due at the time of service. If insurance denies payment, the patient is responsible for the balance. Interest, collection costs, and reasonable attorney fees may be assessed on overdue accounts as permitted by law.

Letter of Authorization (LOA) — Assignment & Release

By signing below, I authorize and direct my insurance company to remit payment for medical benefits directly to the Provider for services furnished. I authorize the Provider to release to my insurer, claims administrators, and their agents any information necessary to substantiate claims for payment, including medical records and billing information. This assignment of benefits does not relieve me of my financial obligations for non-covered services, deductibles, coinsurance, or denied claims.
