Agreement of Financial Responsibility

I, the undersigned patient or responsible party, agree that I am financially responsible for all charges for services provided by the healthcare provider unless otherwise expressly accepted and paid by my insurance carrier. I understand that insurance is a contract between me and my insurer and that the provider may submit claims on my behalf, but that ultimate responsibility for payment remains with me.

I authorize the release of medical and other information necessary to process claims and to obtain payment from my insurance company. I hereby assign benefits directly to the provider for all medical benefits otherwise payable to me, including Major Medical benefits, private or group insurance, Medicare or other payers for services furnished. I understand that assignment of benefits does not relieve me of responsibility for payment if claims are denied, not paid timely, or not covered.

I agree to pay any applicable co-payments, deductibles, co-insurance, and non-covered services at the time of service or within the provider's billing terms. If my account becomes delinquent and is referred for collection, I agree to pay reasonable collection costs, attorney fees, and interest to the extent permitted by law.

Payment Policies

Co-payments are due at the time of service. For procedures requiring prepayment or deposits, the patient or guarantor will be notified in advance. Returned checks will incur a returned check fee. The provider may offer payment plans at its discretion; agreed payment plan terms will be documented separately and remain enforceable. The provider may offset future refunds against outstanding balances.

If insurance information is not provided or is inaccurate, I accept full financial responsibility for charges. I will notify the provider promptly of changes to insurance coverage or billing information.

Authorization and Acknowledgment

By signing below, I acknowledge that I have read and understand this Financial Responsibility Agreement, that I consent to the release of medical information as necessary to facilitate payment, and that I assign benefits to the provider as described above. I understand that I may revoke this authorization only in writing, and such revocation will not affect actions already taken in reliance on this authorization.
