Authorization for Release of Decedent to Funeral Home: The undersigned authorizes the Facility to release the decedent to the custody of the named Funeral Home for transportation, care, and disposition in accordance with the selections above. This authorization permits the Funeral Home to assume responsibility for disposition and to provide any services selected by the undersigned.

Authorization to Release Medical Information: I authorize the Facility to disclose to the Funeral Home and its agents relevant medical and identifying information, including cause of death, diagnostic history, and infectious disease status, as needed for disposition, death certification, or compliance with public health requirements. This authorization is limited to the information reasonably necessary for these purposes.

Indemnification and Liability

Indemnification: The undersigned agrees to indemnify, defend and hold harmless the Facility, its officers, employees, and agents from any claims, liabilities, or expenses arising from the release, transportation, and disposition of the decedent in accordance with this Agreement, except to the extent arising from gross negligence or willful misconduct of the Facility.

Limitation of Liability: Except as required by applicable law, the Facility's liability for any claim relating to the custody, care, or release of the decedent is limited to actual proven damages and shall not include punitive or consequential damages.

Representations and Certifications

Authority to Sign: By signing below, I certify that I am the decedent, the duly authorized representative, next of kin, or agent with legal authority to direct disposition of the decedent's remains and to execute this Agreement. I certify that the information provided in this Agreement is true and correct to the best of my knowledge.

Revocation: This authorization may be revoked by the undersigned prior to the release of the decedent by delivering a written revocation to the Facility and Funeral Home. Revocation does not affect actions taken in reliance on this Agreement prior to receipt of the revocation.
