Emergency Care

In the event of an emergency, Agency will make reasonable efforts to contact the patient’s emergency contact and primary care physician. If necessary, Agency will arrange for emergency medical transport. Patient authorizes Agency to seek emergency medical care if patient is incapacitated and immediate treatment is necessary to prevent serious harm.
