Financial Responsibility and Billing

The Patient acknowledges that certain additions or upgrades may be non-covered by insurance and that the Patient (or responsible party) agrees to pay amounts not reimbursed by the insurer. The Hospital will bill the insurer in good faith; however, the Patient remains ultimately responsible for charges incurred under this Addendum unless written prior arrangement with the Hospital states otherwise.

This Addendum shall commence on the Effective Date stated above and shall remain in effect until the stated expiration date or until services covered by this Addendum have been completed, unless earlier terminated by mutual written agreement. Either party may terminate this Addendum for cause upon written notice to the other party if the other party materially breaches its obligations and fails to cure the breach within a reasonable period. Termination does not relieve the Patient of financial obligations for services rendered prior to termination.

Representations, Warranties and Limitations

The Patient represents that the information provided in this Addendum is true and complete to the best of the Patient's knowledge. The Hospital represents that it will use reasonable efforts to provide or arrange for the additional services described in this Addendum. Except as expressly set forth in this Addendum, no additional warranties or guarantees are made. The Hospital's liability for any claim arising out of or related to services provided pursuant to this Addendum shall be governed by the limitation of liability provisions contained in the underlying admission agreement unless expressly modified in writing in this Addendum.
