By signing this document the patient authorizes billing to the payment method above for hourly care charges, as permitted by the provider's billing policies. The patient is responsible for copayments, deductibles, and any non-covered services. Cancellation within less than 24 hours of a scheduled visit may result in a cancellation fee equal to the minimum charge or one hour of service at the hourly rate, whichever is greater.

RISKS, CONSENT & LIMITATIONS

The patient consents to receiving non-invasive personal care services delivered on an hourly basis. The patient acknowledges that risks may include, but are not limited to, allergic reactions, falls, and unforeseen medical events. The patient affirms they have disclosed all pertinent medical history and current medications. The caregiver and agency will not perform activities reserved for licensed healthcare professionals unless otherwise specified in a separate written order from the patient's treating provider.

The patient retains the right to withdraw consent at any time by providing written notice. Withdrawal of consent does not relieve the patient of financial obligations for services already provided. The provider will document and report any incidents, injuries, or suspected abuse to appropriate authorities consistent with legal obligations.

HIPAA & PRIVACY ACKNOWLEDGMENT

I acknowledge receipt of the provider's Notice of Privacy Practices describing how medical information about me may be used and disclosed. I authorize the provider and its staff to use and disclose protected health information for treatment, payment and healthcare operations consistent with applicable law. I understand I may request restrictions on certain uses and disclosures; such requests must be submitted in writing and will be considered but are not binding on the provider in all circumstances.
